Fowler Wellness Center Membership
To obtain a Fowler Wellness Center Membership:
1) Complete and return a membership form and liability waiver.
a. Forms can be downloaded from our website, or you may visit the grade school office to pick up forms.
b. Mail completed forms to the district office, or drop them off at the grade school office.
2) Review the Fowler Wellness Center rules.
3) Make your first payment. You can pay a monthly membership fee, or an annual membership fee.
4) Get your new key card.
a. All key cards/key fobs issued by the Fowler Recreation Commission will be shut off January 23, 2023. You must obtain a new membership through USD 225 to gain access to the updated facilities.
